Thanks to everyone for their input -- the suggestion to use Cafepress
was an excellent one, unfortunately we already ordered 1000 CDs.  I will
keep this in mind next time.   

I think ultimately, I will just be using one of PayPal's Buy Now buttons
or something like it.  

I've finally decided that setting up an entire store for a single item
is just overkill.
