[wplug-board] updated meeting expense report and instructions

Folks,

I updated the meeting expense report slightly.  I just rearranged a few
things, added more spots for signatures, and put a spot to record total
number of members in attendance.  You can find the forms at:

Also, at the board meeting I was told to make an instruction sheet for
