I setup phpsurveyor (www.phpsurveyor.org) for an evaluation project just the other day.  It was relatively easy to get setup and has all the features you mentioned looking for. 

My only gripe was the look and feel of the survey.  They use templates, which are basically a directory of partial html files that are pieced together to form the survey page.  They use a lot of inline html formatting, which was kind of annoying with a client that changes their mind often enough.  I just edited all the files to run off a single CSS file.  It wasn't terribly difficult, but I think it's worth mentioning.

WPLUG is considering doing a survey of its members to better identify 
the ways we can serve you.  In order to maximize responses, we'd like to 
do this online.  Does anyone have suggestions of decent survey software? 
  I'm talking about more than a poll here.  We'll have multiple types of 
questions (free text, pick one, pick many), and probably some 
conditional logic too.  So, what are the options out there?  I'd rather 
not spend the money on a SurveyMonkey account if there is something Free 
out there.
