Ok, it appears to be a LVM problem:

vgscan -- found active volume group "bar_data"
vgscan -- found inactive volume group "foo_data"
vgscan -- volume group "foo_data" reuses an existing logical volume number;
please vgexport/vgimport that VG or use option -f
