Sometime in May Greg Akins assaulted the keyboard and produced:

Since we're talking about selling one cd here how about you put a web page
up with a link to paypal. They can enter their address and stuff at paypal,
and you can put a note on the page for flat rates on shipping ($3 anywhere
in the U.S). I would imagine what you need to know is buried in this page
somewhere:
