Thanks to all for the help.
To answer your questions:
Yes, I was root.
No, another package manager was not open.
Did not try the up-to-date module. Need to find out what it does.
The packages were build for FC4.
Creating an empty __db.000 file seems to have worked. I installed two
packages last night and they are both working.
I ran rpm --rebuilddb and found that it removed all of the __db.00x files.
I hope this does not prevent further package installations.
