Good questions, although none are really addressed by this document.  
You'll want to read up on WIPO itself for the details of how this 
works--it's a bit complex for a quick response, but it's basically a 
carrot-and-stick mechanism. 

Also understand that this proposal isn't being made in a vacuum:  WIPO 
is an existing body that the proposal is attempting to reform.
