Also check to see if you've explicitly told MSN Messenger in its preferences that it is behind a firewall.  A was messing around with iptables rules for hours one day to try and get Yahoo Messenger vid & voice chat working for my dad behind my firewall, to no avail.  How did I fix things?  I went into Yahoo's preferences and removed the setting that told it it was behind a firewall.  Worked like a charm, and Yahoo Messenger was none the wiser.  I don't know if it's the same way with MSN Messenger though--but a heads up for ya just in case.
