If you disable the pnp feature of the card, then you must configure its
IO port and IRQ number. These would be fixed, and that is what you
should use with your setserial, or whatever else you use to set it up.
I'd suggest you use the default values for the first or second serial
ports and let the boot scripts find it.

Also, make sure your BIOS has the IRQ of the modem set as "Legacy ISA" 
or equivalent.
